II. Notices to the Consumer

1. Fair Credit Reporting Act: The information collected on this form will be submitted to a participating lender(s) for consideration as to whether it meets their requirements for loan approval. Upon request I will be advised as to actual names and locations of lenders.

2. Credit Report: A Credit Report may be obtained in connection with this application. Upon Request, I will be told if a credit report was obtained and given the name and address of the credit reporting agency providing the report

3. Credit Investigation: I authorize a credit investigation based on the information voluntarily provided herein, which I attest is true and complete.
